MINOR PHOTO RELEASE FORM

I hereby grant my permission to the Regional District of North Okanagan (RDNO), and those acting with its permission, or its employees, the permission to take photographs of the below minor in relation to Regional District of Okanagan during soccer activities and events for the purpose of creating a photo gallery accessible to members of Lumby Soccer Club.

I understand that I will not be paid for these photographs and have no rights to them. I hereby waive any right to inspect or approve the finished photograph or advertising copy or printed matter that may be used. I release the RDNO, its officers, employees and agents, from any and all claims of harm and liability as a result of any distortion, blurring, or alteration, optical illusion, or use in composite form, either intentionally or otherwise which may occur from making, showing, using or distributing these photographs/video.
I understand that these photographs, will be made available in a gallery format accessible to other members of the soccer club.

Lumby and Area Recreation Facilities and Rentals

Looking for an event venue? Choose from one of the many spaces available within White Valley.  We can accommodate everything from small intimate meetings to larger halls that are perfect for workshops, private events, or weddings. These rooms can be set up in a variety of configurations to suit your needs at the White Valley Community Centre.  In addition,  Pat Duke Arena can be booked for tradeshows, sporting events, or concerts. The ice and dry floor arena, tennis courts, sports fields are also available for private rentals.

All bookings require the appropriate insurance and license.  If you are booking a social event at one of our facilities or parks you will require the appropriate insurance and licenses. This includes liability insurance, liquor licences, and permits.
